Anterior Shoulder Pain: Diagnosing Biceps Tendonitis and Relieving Inflammation at Home
Sharp pain in the front of the shoulder when reaching overhead, pulling on a coat, or lifting a grocery bag is frequently misdiagnosed as a routine rotator cuff strain or mild arthritis. Hallmark Symptoms of Biceps Tenosynovitis
Pain Distribution: Well-defined ache along the anterior or anterolateral shoulder, frequently radiating distally into the biceps muscle belly or towards the elbow crease.
Mechanical Provocation: Sharp aggravation during resisted elbow flexion, forearm supination (turning the palm upward), and active arm elevation above shoulder height.
Auditory & Tactile Sensations: Audible clicking, snapping, or crepitus in the front of the shoulder caused by the swollen tendon subluxating in the groove.
Progressive Weakness: Functional weakness and premature fatigue when lifting or carrying objects.
Nocturnal Discomfort: Inability to sleep on the affected side due to direct pressure on the inflamed bicipital groove.
Why Topical Anti-Inflammatory Ointments Fall Short
Over-the-counter NSAID creams and balms penetrate only 1–3 mm into the stratum corneum. The long head of the biceps lies deeply beneath skin, subcutaneous fat, and the thick anterior fibers of the deltoid muscle.
